Carevera Membership Privacy Notice

Carevera collects the information in the membership application to evaluate and administer membership, confirm service-area eligibility, communicate with applicants and members, coordinate member services and handle payment administration.

Information collected

The form requests identity and contact information, date of birth, service address, emergency-contact information, plan and payment preferences, and optional notes. Applicants should not submit detailed diagnoses, prescriptions, laboratory results or other medical records through this general form.

Health information

If health information is later needed for care, Carevera should collect it through an appropriate protected process with the applicable lawful basis, privacy notice and security controls.

Sharing

Information may be accessed by authorized Carevera personnel and necessary service providers for legitimate membership administration, payment processing, communications or care coordination, subject to appropriate confidentiality and security measures. Carevera will not sell membership personal information.

Retention

Carevera will retain information only as long as reasonably necessary for membership administration, legal and accounting requirements, disputes and legitimate recordkeeping, after which it should be securely deleted or anonymized as appropriate.

Marketing

Marketing consent is optional and separate from the consent required to process a membership application. It may be withdrawn through Carevera's published contact channels.
